Hours per fortnight: 32 hours available per fortnight for internal applicants to pick up additional ongoing hours.

Kyneton Health - Acute
The Registered Nurse is an integral member of the multidisciplinary team who works to achieve a high standard of evidence based nursing care to the patient/client and significant others for positive treatment outcomes.
You will undertake and contribute to the multidisciplinary assessment treatment and discharge planning of patients and assumes accountability and responsibility for his/her own practice based upon his/her level of educational preparation and competence.
Responsibilities & Major Activities
Provide patient-centred nursing care including recognition of normal and abnormal in assessment, intervention and evaluation of individual health and function status. Work collaboratively with all members of the multidisciplinary team to facilitate integrated person-centred care leading to timely and effective client discharge. Monitor the impact of nursing care and maintain ongoing communication with the nurse in charge regarding the health and functional status of individuals. Use critical and reflective thinking skills in contributing to decision making which includes reporting changes in health and functional status and individual responses to health care interventions. Support the development of and comply with organisational and service level policies and procedures pertinent to registered nursing practice. Contribute to the review of care plans in conjunction with individuals/groups. Implement planned nursing care to achieve identified outcomes. Ensure communication, reporting and documentation are timely and accurate. Organise workload to facilitate planned nursing care for individuals and groups. Prioritise the delivery of nursing care to individuals/groups.

About you:
We seek applicants with a minimum of 2 years experience in Acute wards.

The Community Mental Health Worker role is to provide support and services for clients experiencing persistent and enduring mental illnesses and also those individuals and organisations who provide support to the client. The Community Mental Health Worker will work with individuals and their supports to achieve optimal overall health and wellbeing utilising contemporary best practice in mental health service delivery.
Key Responsibilities
* Provide current evidenced-based and best practice mental health assessments and interventions in accordance with professional standards and the National Mental Health Commission “Equally Well Framework” * Undertake comprehensive psychiatric assessments, identifying and prioritising the needs of the client and their support networks * Develop, implement and regularly review care plans with clients, which include their physical and mental wellbeing within a broader social model of health * Provide case management services where deemed necessary * Ensure clients receive a high quality service which is delivered in a timely and professional manner
* Provide referrals to external organisations and internally to other CHRH services where necessary to achieve optimal outcome for the client * Engage with the clients support systems e.g. family, friends, kinship networks * Engage with key service providers such as but not limited to General Practitioners, other treatment providers, housing, community, vocational, education and recreation services * Maintain regular and ongoing communication with other mental health service providers involved in shared care * Provide secondary consultation to other community-based organisations * Maintain accurate and current medical records * Ensure data is collected accurately and reported using the reporting system prescribed by the funding body and other organisational data systems * Participate and contribute to team and organisational operations * Maintain and advance skills and knowledge through participation in ongoing learning and clinical supervision * Identify opportunities for quality improvement and engage in their delivery * Other duties consistent with the role as may be reasonably directed by the Manager.

Kyneton Acute
The Associate Nurse Unit Manager is an integral member of the management team, working with the Nurse Business Manager to ensure efficient operation of the clinical environment and is responsible for the delivery of quality care within the Acute Unit. The ANUM manages the nursing and related resources available to the department, sets the clinical standards to achieve a high quality of nursing care and fosters and maintains a learning environment for staff, committed to professional development. The ANUM is appointed to undertake in-charge and shift leadership functions during the off-duty periods of the NUM.
This role will also involve regular night duty in the role of After Hours Coordinator (AHC). In the absence of the Chief Executive, the Director of Nursing and the Nurse Managers AHC is responsible for the overall management of the Kyneton Health. The AHA will support the Executive and the Nurse Managers in the provision and maintenance of quality care within CHRH, through competent clinical and business practice, effective communication and active participation. The AHA will be a highly motivated leader, responsible for the maintenance of safety for patients, visitors and staff.

We seek applicants with experience in acute nursing, including palliative care and medical / surgical nursing. Applicants must have strong time management skills.
